5.3 To Comply with Legal and Regulatory Obligations

5.4 To Administer Operations and Improve Services

• After the retention period, records are securely destroyed in accordance with our record-retention policy and industry best practices.

• Paper records are stored in locked, access-controlled areas; access is restricted to authorized personnel only.
• Electronic records (EHR, billing software, email, file servers) are protected by encryption, password controls, firewall protection, and intrusion-detection systems.
• Workstations and mobile devices that access PHI are password-protected and configured to lock after periods of inactivity.

8.3 Administrative Safeguards
• We conduct regular staff training on privacy and security protocols, HIPAA compliance, and incident reporting.
• We perform periodic risk assessments and security audits to identify and remediate vulnerabilities.
• Access to PHI is granted on a “need-to-know” basis; each Staff member is assigned role-based permissions.

8.4 Data Breach Response
• In the event of a suspected data breach involving PHI or other sensitive personal data, we will promptly investigate and, if confirmed, notify affected individuals, the U.S. Department of Health and Human Services (in accordance with HIPAA rules), and state authorities as required by law, within the mandated notification timelines.
• Our Breach Response Team will take immediate steps to contain the breach, mitigate harm, and implement corrective actions to prevent recurrence.

9. Use of Cookies, Tracking, and Analytics
9.1 Cookies and Similar Technologies
• We use cookies and web beacons on our website to remember user preferences (e.g., language, form entries), provide secure login sessions, and compile anonymous analytics.
